Metabolic Health Explained: The 5 Numbers Every Indian Should Know
Most people think of health as "no disease" or "a normal weight." Metabolic health is more precise. It describes how well your body handles energy: how it processes sugar and fat, regulates blood pressure, and stores weight. You can look slim and still be metabolically unwell, and you can carry extra weight and still have good numbers. That is why a weighing scale alone tells an incomplete story.
What does "metabolically healthy" mean?
A person is generally considered metabolically healthy when five markers sit in a safe range. When three or more are off, doctors diagnose metabolic syndrome, which significantly raises the risk of type 2 diabetes, heart disease, fatty liver, and stroke.
The 5 numbers to know
1. Waist circumference. For Indians, abdominal obesity starts at about 90 cm in men and 80 cm in women. Belly fat is more metabolically active than fat elsewhere.
2. Fasting blood glucose. Below 100 mg/dL is normal. 100 to 125 mg/dL is prediabetes. 126 mg/dL or higher on repeat testing suggests diabetes.
3. HbA1c. This reflects your average sugar over about three months. Below 5.7% is normal; 5.7% to 6.4% is prediabetes; 6.5% or above indicates diabetes.
4. Blood pressure. Aim for below 130/85 mmHg at the very least; many doctors prefer below 120/80.
5. Lipids. Triglycerides below 150 mg/dL, and HDL ("good" cholesterol) above 40 mg/dL in men and 50 mg/dL in women.
Why Indians need to watch these early
Studies consistently show that South Asians develop insulin resistance, diabetes, and heart disease at lower body weights and younger ages than many other populations. Waiting for a high BMI before getting tested means many people are caught late. If you have a family history of diabetes or heart disease, start checking in your late 20s or early 30s.
How to check your numbers
A basic metabolic panel (fasting glucose, HbA1c, lipid profile, liver and kidney function) plus a waist measurement and blood pressure reading covers most of it. It takes one morning. Your doctor may add fasting insulin, uric acid, vitamin D, or an ultrasound or FibroScan for fatty liver depending on your risk.
What to do if your numbers are off
The good news: metabolic health is highly responsive to change. Modest weight loss of 5% to 10%, regular strength and aerobic exercise, better sleep, less ultra-processed food, and a protein- and fibre-adequate diet can improve most of these markers within months. Some people also need medication, and that is not a failure. It is part of treatment.
FAQs
Can I be metabolically unhealthy at a normal weight? Yes. This is sometimes called "thin-fat" or "normal-weight metabolic obesity," and it is common in India.
How often should I get a metabolic check-up? Once a year if your results are normal, and every 3 to 6 months if you are making changes or have abnormal values.
Is a high HbA1c the same as diabetes? Not always. 5.7% to 6.4% is prediabetes, which is often reversible with lifestyle change.
